1. This application must be executed by a
majority in interest of the partners or by one or more of the partners
authorized by a majority in interest of the partners.
2. This application must be accompanied by

4. Registration as a Registered Limited
Liability Partnership is effective when the Secretary of State files this
application, as indicated by the date stamped in the box on the other side of
this document. If the application is properly completed and submitted with the
correct filing fee, the Secretary of State will file the application and return
the conformed copy stamped with the effective date of filing.
5. Registration of a Registered Limited
Liability Partnership is effective for one year after the date the registration
is filed, unless it is voluntarily withdrawn before then by filing with the
Secretary of State a written withdrawal notice executed by a majority in
interest of the partners or by one or more partners authorized by a majority in
interest of the partners. Registration must be renewed annually. The
partnership may renew a registration by filing an application for renewal prior
to the expiration of the effective term of registration. The partnership should
arrange its own reminder of the renewal deadline, since the Secretary of State
is not obliged to send renewal notices.
